The development of organ transplantation as a widely available and clinically effective
therapy has provided an enormous benefit for the care of patients with end-stage renal
disease (ESRD). One-year cadaver donor graft survival rates have improved from 52% to
nearly 90% between 1977 and 1998 (1, 2). Such outcomes have encouraged more patients
